At Wayne High School, our Athletic Department is dedicated to creating  a culture of excellence, teamwork and sportsmanship. We strive to provide a safe and inclusive environment where student athletes can develop leadership, character and their physical skills. Our mission is to inspire every student athlete to reach their fullest potential both in the classroom and on the field. We celebrate diversity and encourage all students to participate regardless of skill level as we build champions in life and in sports.


IHSAA Physicals: The IHSAA By-Laws establishes April 1 as the date for eligibility for new physicals. This means, all athletes wishing to participate in the 2024-25 school year must have a physical completed on or after April 1, 2024. 

All parents and student athletes have to create an account on Final Forms. Once parents and athletes create their account, the only requirement is to follow all the steps on Final Forms and upload the IHSAA physical. Once accounts have been created, parents will access their child’s Final Forms account and verify and update information.

  1. Parents and Athletes create an account and complete all the steps
  2. Parent uploads the IHSAA Physicians Examination page to Final Forms
  3. Athlete completes all required forms via Final Forms
